School-Based Health Centers sponsored by Henry Ford Health System are the place for teenagers and young adults to go for health problems big or small.
We’re Convenient, Professional and Affordable!
If you’re like most teens, you feel great most of the time. Occasionally, small health problems occur and these problems can be warning signs of a temporary illness or a serious disease. Sometimes illnesses can be caused by problems like poor diet or too much stress-conditions that can damage your health for life.

The Henry Ford Health System School-Based Health Initiative believes that both mental and physical health are essential for a student’s success. Our health centers strive to provide comprehensive, interdisciplinary, and confidential health services that will meet the needs of students in an atmosphere that is caring and provides mutual respect.
Access to Services
Services are available to youth ages 10 years thru 21 years who reside in Center Line, Warren Woods, Fitzgerald and Van Dyke Public Schools.
If you are under 18 years of age, a consent form must be signed by a parent or guardian. The consent form must be renewed annually with updated health history and insurance information. A copy of the student’s vaccine record also needs to be submitted and will be kept on file as well as entered into MCIR, the Michigan state vaccine computer registry.
Emergency Care, Crisis Intervention, and Michigan Minor Law criteria exemptions do not require parental consent. (See Criteria on Consent Form)
All services are confidential and are offered for a low cost or no charge.
All students are eligible to receive health care regardless of their insurance situation. The health center accepts all insurances. Third party insurance will be billed for appropriate services through the School-Community Health Alliance of Michigan billing service.
If the student has no health insurance the Fitzgerald Health Center can assist with an online Medicaid application to determine eligibility and obtain insurance, if eligible. At this time, there are no out-of-pocket expenses billed to the student for services.

Resources: The Fitzgerald Health Center partners closely with the Fitzgerald Public School district and Fitzgerald High School, in which the health center is located.
The Henry Ford Health System Fitzgerald Health Center works with a number of community health agencies in the provision of both physical and mental health services for individual students as well as health education programs within the school.
The current agencies we partner with include: Michigan Department of Community Health, Michigan Department of Education, Macomb County Health Department, CARE of Macomb, Turning Point, Henry Ford Hospital Trauma Nurses, The MINDS Program, Alateen, Mobile Dentists, YMCA Macomb County Abstinence Partnership, and Henry Ford Macomb Hospital – Warren Campus.
